Text For Citation: 01 Item/Group: 007 Hazard:
29 CFR 1910.147(e)(3): Each lockout or tagout device shall be removed from each energy isolating device by the employee who applied the device: On or about 26 April 2017, The employer removed a personal lockout device from the plastic compactor near door #11 without first taking the following safety concerns to ensure the safety of the authorized person before the following work day: a. Making all reasonable efforts to contact the authorized employee to inform him/her that his/her lockout or tagout device has been removed; and b. Ensuring that the authorized employee has this knowledge before he/she resumes work at that facility. This exposed employees to amputation and/or crushing injuries from unexpected start-up of the compactor. Abatement documentation is required for this item in accordance with 29 CFR 1910.19(d).
