29 CFR 1910.178(l)(2)(ii): The employer does not ensure that each operator successfully completes the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Location: Work Floor The following violation was observed on November 28th, 2016: 1) The employer did not ensure that an employee had successfully completed forklift training consisting of a combination of formal and practical training. 2) The employer did not evaluate the driving performance of the employee at the facility . To abate this violation, the employer must ensure that all forklift divers, including temporary employees who operate forklifts, have current forklift training, including practical training of operations at the facility. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance
