29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: North, polyester department, trimming and grinding work station: on or about November 8, 2016, employees were exposed to a fire hazards where a relocatable power tap was connected in series to another extension cord and used for powering several appliances that had their plugs taped or rubber banded to the receptacles. This device was not listed and labeled for such use at an industrial manufacturing site. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
