29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. On or about September 16, 2016, and times prior thereto, in the Mill area near the Glue Machine where employees were exposed to electrical shock, tripping, and fire hazards when cleaning around a metal electrical outlet box on the floor area that was not used as listed and labeled. b. On or about September 16, 2016, and times prior thereto, in the Mill area near the CNC where employees were exposed to electrical shock, tripping, and fire hazards when cleaning around a metal electrical outlet box on the floor area that was not used as listed and labeled. c. On or about September 16, 2016, and times prior thereto, in the NW Mill area where employees were exposed to electrical shock hazards when operating equipment plugged into a local manufactured handy box that was not used as listed and labeled. d. On or about September 16, 2016, and times prior thereto, in the NW Mill area where employees were exposed to electrical shock hazards when operating an Angle Borer with a side-mounted metal outlet box that was not used as listed and labeled.
