29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure employees were protected while pulling six 480V fuses on the 12 yard Plant Cement Mixer. Employees are required to pull the fuses on the mixer, and did not verify zero energy with a meter, before performing work and did not wear the required face, eye, hand, and body protection, exposing them to electrical shock and arc flash hazards when pulling the fuses with fuse pullers.
