29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about July 7, 2016, the employer failed to develop, document, and utilize procedures for the control of hazardous energy when employees were engaged in activities covered by this section. Employees are exposed to caught in and electric shock hazards in that one a single energy control procedure is listed as covering "all" collar/collatch machines. At least three models, including both automatic and manual types of collar/collatch machines are in use in the facility. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
