(b) Location - Machine #171: On or about 3/23/16, the employer failed to provide point of operation guarding on a Nakamura CNC Lathe. (c) Location - Machine #63: On or about 3/23/16, the employer failed to guard rotating parts on a manual Mori-Seiki lathe.
