29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) Grinder Area; On or about March 15, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to walk and work in close proximity of an unguarded projecting shaft end that projected more than one and half times the diameter of the shaft on the conveyor bed at the grinder. b) #2 Board Trimmer; On or about March 15, 2016 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to walk and work in close proximity of an unguarded projecting shaft end that did not present a smooth edge and end and projected more than one and half times the diameter of the shaft.
