29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Fabrication Shop; On or about February 17, 2016 and at times prior thereto, an employee was exposed to electric shock hazards in that an employee was permitted to use a De Walt brand portable band saw to cut square steel tubing that was plugged into an extension cord consisting of a cord and metallic outlet box, designed for fixed installation and was not securely affixed to any surface.
