29 CFR 1910.272(m)(4): The employer did not implement procedures in the grain handling facility for the use of tags and locks which would prevent the inadvertent application of energy or motion to equipment being repaired, serviced, or adjusted, which could result in employee injury. A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016 and times prior thereto: Employees performing servicing and maintenance activities on equipment including, but not limited to, the corn cracker/shaker bed and the bird seed mixer. The company's general lockout tagout procedure does not include elements including, but not limited to, pre-service/maintenance notification, verification of energy isolation, and post-service/maintenance notification and inspection following the work and removal of the lock/tag.
