29 CFR 1926.453(a)(2): Aerial lifts were "field modified" for uses other than those intended by the manufacturer, the modification had not been certified in writing by the manufacturer or by any other equivalent entity. Modification did not conform with all applicable provisions of ANSI A92.2-1969: a) On or about 8/27/15 at a demolition project located at 1111 Allen Street, Jamestown, New York; employees modified a Genie Lift S-65 by using a pallet to enclose part of the basket to carry debris from the bridge. The lift also was used as a cart to carry, support and store the oxy-acetylene cylinders and torch set. b) On or about 11/13/15 at a demolition project located at 1111 Allen Street, Jamestown, New York; employees modified the intended use of a Genie Lift S-65 by using it to carry and lower structural steel from the bridge. The lift also was used as a cart to carry, support and store the oxy-acetylene cylinders and torch set. ABATEMENT CERTIFICATION REQUIRED
