On or about July 20, 2015, Establishment: The employer operated processes using 18,609 pounds of acetone and 15,378 pounds of isopropyl alcohol 99%. The employer had not compiled written process safety information including block flow diagrams, maximum intendent inventory or consequences of deviation.
