29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. Line 1 - Zn Chloride Line - On or about June 16, 2015, employees had access to an electrical box with open covers that had energized wires that were exposed and protruding outside the box on the south wall post, in an aisle employees use to access equipment, thereby exposing employees to shock and electrocution hazard. b. Line 2 - Zn Chloride Plate Line - On or about June 16, 2015, employees had access to a control boxes that had energized wires that were exposed and protruding outside the box, near the dryers, thereby exposing employees to shock and electrocution hazards. c. Chemical Storage Area - On or about June 16, 2015, employees had access to a control box with a missing start button that had energized wires that were exposed, thereby exposing employees to shock and electrocution hazard.
