29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from the recognized hazard of damaged insulation. Forge Shop, Wyman and Gordon, 244 Worcester Street, North Grafton, Massachusetts; Extension cords were in use with damage to the outer jackets and some with exposed, energized conductors, exposing employees to electric shock hazards from the 120 VAC circuits. a. the cord used to charge the JLG lift had exposed conductors b. the cord used to charge Instrument Department Golf Cart #3 had exposed conductors c. the cord used to power the spotlight for PIT repairs in the hut had exposed conductors after a forklift was driven off with the light attached and pulled the wires free from the light.
