29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about 4/28/2015, in the kitchen area; employees were exposed to fall hazards where two (2) 36 x 36 inch square x seven (7) inches deep holes and five (5) 16 x 16 inch square x six (6) inch deep holes were not filled in a dimly lit room. Employees are required to walk in and around the room to perform masonry operations.
