29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a/ Ambrosia Fragrance/Piscataway NJ Site: Production Areas. All hard-wired machinery/equipment and all cord/plug connected equipment where the employee does not have exclusive control over the cord/plug at all times when providing maintenance/service. The lockout program provided did not include but not limited to the following: 1/ the program was not documented and tailored to the site specific equipment to be worked on 2/ there were no specific steps documented to de-energize the equipment 3/ means to control the use and identity of locking devices 4/ designated personnel involved in the program, etc. Equipment include but not limited to: Filling Machines, Packaging Machines, etc. Observed on/or about 04/21/2015.
