29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: a. On or about 4/20/2105 at the Queen City 24 inch grinder; employees performing sanding operations on and working around the Queen city grinder were exposed to amputation and struck-by related injuries where the left and right side disks were not guarded around the outer surface of the disks and other rotating parts. b. On or about 4/20/2015 at 2200 Stephens Street, Tucker GA; employees performing press operations were exposed to struck-by hazards where the light curtains on presses including but not limited to the Tranemo 1000 ton, partial revolution, hydraulic press # 18, were not tested with a test rod: 1. employees used their fingers to test the light curtains. 2. employees relied on the computer to detect faults
