29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. 1.) On or about April 15, 2015, the employer did not implement procedures for rescuing entrants from permit required confined spaces in the event of an emergency or for providing emergency services to the entrants. Employees are entering the Trans Mat parts washer to conduct cleaning and preventative maintenance. Employees are exposed to caught in hazards from the chain conveyor, chemical hazards from chemicals such as but not limited to; Potassium Hydroxide and Phosphoric acid and water engulfment.
