29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a. On or about March 3, 2015, no ammonia detection system had been installed in the ammonia engine room in accordance with ANSI/IIAR-2, section 13.2 and the ANSI/ASHRE Standard 15 paragraph 8.13.2 requirements for ammonia refrigerant detections systems capable of providing warning alarm signaling and ventilation actuation. b. On or about March 3, 2015, no alarm system was installed in the ammonia engine room to alert potentially exposed employees of an ammonia release as required by ANSI/IIAR-2 paragraph 13.2.1. c. On or about March 3, 2015, the walls and doors of the ammonia engine room were not tight fitting as required by ANSI/IIAR-2 section 13.1.1.3. d. On or about March 3, 2015, there were no eyewash or deluge showers outside the ammonia engine room exit doors as required by ANSI/IIAR-2 section 13.1.6.1. e. On or about March 3, 2015, the ammonia engine room exit doors were not equipped with panic hardware door releases as required by ANSI/IIAR-2 section 13.1.10.3. f. On or about March 3, 2015, an emergency shutoff switch was not located outside the ammonia engine room exit doors as required by ANSI/IIAR-2 section 13.1.13.2. g. On or about March 31, 2015, pressure vessel relief to atmosphere piping discharge points were not in accordance with the ANSI/IIAR-2 standard for Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems section 11.3.6, specifically: 1. At the processing building, the ammonia pressure relief valve piping vent for the spiral freezer's recirculator vessel was located less than 15 feet from personnel work areas and less than 15 feet from and exit door. 2. At the ammonia High Pressure Receiver tanks, the pressure relief valve vents were located at head and shoulder level in a work area less than 20 feet from an exit door and directly beneath the condenser platform walkway. 3. At the processing building, the ammonia pressure relief valve piping vent for the brine chiller surge tank was located less than 15 feet from personnel work areas. 4. At the ammonia purger unit shack, the pressure relief valve vent was located directly inside the shack. h. On or about March 3, 2015, in the ammonia engine room, at the V-2 intercooler, and in the Spiral Freezer machinery room, oil removal fixtures for oil pots were not equipped with self-closing or quick closing valves at the oil drain points as required by the American National Standards Institute/International Institute of Ammonia Refrigeration, ANSI/IIAR-2 Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems section 14.2.3 to prevent inadvertent or uncontrollable release of ammonia following oil drainage. i. On or about March 3, 2015, throughout the 13,000 pound anhydrous ammonia refrigeration system, ammonia piping and valves were not labeled for flow direction or pipe contents as required by Section 10.6 of the ANSI/IIAR-2 standard for Equipment, Design, and Installation of Ammonia Systems and in chapter 11 of the American Society of Heating, Refrigeration, and Air Conditioning Engineers Standard, ASHRAE-15 Safety Code for Mechanical Refrigeration.
