29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): On or about December 11, 2014, employees working performing railcar cleaning tasks were exposed to burn hazards in that flame resistant personal protective equipment was not provided to employees working with flammable vapors and liquids.
