29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a.) HBD/Thermoid, Inc. located at 1301 W. Sandusky St. Bellefontaine, OH; On or about November 13, 2014, the employer did not assure employees were protected while operating the Quality Quest Crimpers. Employees were required to crimp the end of hoses and are exposed to amputation hazards when placing the end of the hose into the crimper.
