29 CFR 1910.215(b)(5): The angular exposure of grinding wheel periphery and sides for safety guards used on surface grinding machine(s) employing the wheel periphery exceeded 150 degrees: On or about October 31, 2014, and at times prior thereto, while operating the surface grinders, employees in the maintenance room and tool room were exposed to struck-by hazards: a) In the Tool Room, the Linear Surface Grinder (Model No. 618/Serial No. 8989) was missing its protective abrasive wheel guard. b) In the Tool Room, the HARIG 612 Surface Grinder (PMT asset No. 089) protective abrasive wheel guard would not lock in place. c) In the Tool Room, the HARIG 612 Surface Grinder (PMT asset No. 25) protective abrasive wheel guard would not lock in place.
