29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: a. East side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had one scaffold leg resting on a wooden pallet, while installing the cinder blocks to the wall. b. South side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had one scaffold leg resting on a stack of boards and another leg on a wooden pallet, while installing the cinder blocks to the wall. c. West side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which had the scaffold legs resting on 4X6 lumber blocks, while installing the cinder blocks to the wall.
