29 CFR 1910.146(e)(1): Before entry is authorized, the employer shall document the completion of measures required by paragraph (d)(3) of this section by preparing an entry permit. a. On or about September 25, 2017, located and Furnace #2 and Furnace #3, the employer failed to document an entry permit prior to authorizing employees to enter the furnaces, thereby exposing the employees to the hazards associate with confined space entry in the furnaces.
