29 CFR 1910.146(d)(10): Develop and implement a system for the preparation, issuance, use, and cancellation of entry permits as required by this section; a. On or about September 25, 2017, located at Furnace #2 and Furnace #3, employees were exposed to injury from the hazards associated with permit confined space entry. The employer failed to develop and implement a system for the preparation, issuance, use and cancellation of entry for employees entering furnaces.
