29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) On or about 12/9/2014, the M4-A in feed belt chain and sprocket drive guard had a hole in it which exposed the ingoing nip for the chain and sprocket. The nip point was approximately 1/2" inside of the guard. b) On or about 12/11/2014, the Salwasser case packer was provided with a 16" high platform that allowed access to an unguarded chain and sprocket approximately 18" inside of the equipment.
