29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o): a) On or about 12/9/2014 the ingoing nip point was exposed on a belt and pulley for RAPISTAN conveyor in tube discharge area. It was approximately 6" from the frame of the conveyor to the ingoing nip point. b) On or about 12/11/2014 the ingoing nip point on a belt and pulley for the 1st pin wheel drive was exposed. It was approximately 6" to the nip point from the frame of the machine which had a 4" wide by 10" high opening in it. c) On or about 12/11/2014 the Facial upright cartoner had an opening in it approximately 21" by 21" which allowed access to an unguarded belt and pulley ingoing nip point approximately 11" inside of the equipment.
