Paper Machine #9- Employees were not protected from a fall hazard where the broke hole on the drive side of the paper machine was not protected with a guardrail. b. Paper Machine #9- Employees were not protected from a fall hazard where the broke hole near the bottom felt loop roll, did not have a guardrail on the drive side and did not have mid-rail on the tending side of the paper machine.
