29 CFR 1910.146(d)(6): Provide at least one attendant outside the permit space into which entry is authorized for the duration of entry operations: a. On or about February 28, 2018, located at the Banbury, the employer did not ensure that at least one attendant was positioned outside a permit-required confined space where employees entered a Banbury Pit in order to perform daily cleaning, service and maintenance to equipment, and to remove fallen rubber batches from an overhead conveyor.
