29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) On or about 12/9/2014 the ingoing nip point was exposed on a belt and pulley for RAPISTAN conveyor in tube discharge area. It was approximately 6" from the frame of the conveyor to the ingoing nip point. b) On or about 12/11/2014 the ingoing nip point on a belt and pulley for the 1st pin wheel drive was exposed. It was approximately 6" to the nip point from the frame of the machine which had a 4" wide by 10" high opening in it. c) On or about 12/11/2014 the Facial upright cartoner had a 21" by 21" opening in it that allowed access to an unguarded belt and pulley approximately 11" inside of equipment.
