29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) Welding Area - The employer did not visually inspect equipment before use as evidenced by a yellow extension cord where the outer jacket had separated from the cord's plug end. Employees had this cord plugged into a 15-amp six-outlet current tap, plugged into the two-outlet receptacle box (located to the left of the exit door where the natural gas line enters the building), for use in powering equipment such as a portable grinder, exposing employees to electrical and fire hazards, on or about May 6, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
