29 CFR 1926.1431(f)(3)(i): Materials and tools were not secured to prevent displacement: (a)Henry Carlson Construction, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the controlling employer did not ensure subcontracted employees were protected from falls, crush, and struck-by hazards in that materials and tools being hoisted on the Doka work platform were not secured to the platform. This condition exposed employees to fall, crushing, and struck-by hazards. (a)Henry Carlson Construction, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the controlling employer did not ensure employees and subcontracted employees were protected from falls, crushing, and struck-by hazards in that, formwork, metal box of ties, and associated equipment being hoisted on the Doka work platform were secured to the platform. This condition exposed employees and subcontracted employees to falls, crushing, and struck-by hazards.
