Durable Corporation on or about October 13, 2021, at the address of 75 N. Pleasant St. N. Norwalk, OH: The employer did not ensure the flywheel was effectively guarded on the Alva Allen Industries model BT12 12 ton full revolution press. In this condition, employees operating on or near the flywheel were exposed to rotating parts and caught in hazards.
