29 CFR 1910.1200(h)(3)(i): Employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area: On or about November 11, 2020, an explosion occurred during the manufacturing of PureVista a water-activated product used to generate chlorine dioxide and the employer did not ensure that employees and management officials were provided effective training on hazards to which they were exposed. Effective training lacked one or more of the following elements: methods and observation used to detect the presence or release of the hazardous chemical, as signs of a combustible dust fire. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
