29 CFR 1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used. The employer does not ensure hooks fabricated in house and used to hoist/move wheels for the saws are rated with for their lifting capacity. This violation was observed on or about January 16, 2019, in the facility, exposing employees to struck-by hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps that is taking to ensure hooks are rated for safe lifting capacity.
