29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: Site- Employees were not protected from electric shock where a branch circuit, used to feed a wall fan adjacent to the panel saws, was disconnected from the fan and had exposed conductors. The branch circuit was deenergized at its disconnect but a lock and tag was not applied to the disconnect.
