29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: a) Ethylene Oxide Continuous Plant, Croda-Atlas Point. - On or about November 25, 2018, the employers process hazard analysis failed to address human factors relating to the confusing displays on the Control Room Distributed Control System user-machine interface (e.g., Human Machine Interface) for activating the firewater deluge system resulting in operator errors, such as the erroneous activation of incorrect deluge systems during an emergency release, contributing to employee exposure to ethylene oxide.
