29 CFR 1910.146(c)(4): If the employer decides that its employees will enter permit spaces, the employer shall develop and implement a written permit space program that complies with this section. The written program shall be available for inspection by employees and their authorized representatives: a. On or about February 28, 2018, located at the Curing Room and Mandrel Hose "Geese" Curing Rooms, the employer did not develop and implement a written permit space program for employees that entered a permit-required confined space in the 16 autoclaves in order to insert and remove mandrel carts. b. On or about February 28, 2018, located at the Banbury, the employer did not develop and implement a written permit space program for employees that entered a permit-required confined space in the Banbury Pit in order to perform daily cleaning, service and maintenance to equipment, and to remove fallen rubber batches from an overhead conveyor.
