29 CFR 1926.1053(b)(15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. Location: Mass Avenue & Locust Street, Lexington, MA: On or about 9/11/2017, the bent/damaged eight-foot fly section of an aluminum extension ladder used by the employee to access/egress the trench was not periodically or after any occurrence that could affect its safety inspected by a competent person.
