29 CFR 1926.1204(c): Under the permit-required confined space program required by 29 CFR 1926.1204(c), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations including, but not limited to, items (c)(1) through (c)(8) of this paragraph: a) 9000 River Rd, Delair, NJ, 08110- Anodizing: The employer did not develop and implement the means, procedures and practices necessary for safe entry into anodizing tanks for employees conducting electric arc welding including, but not limited to, acceptable air quality parameters, procedures to eliminate drowning hazards, to eliminate falling hazards, determine the frequency of air monitoring when in the tank to determine acceptable atmospheric conditions and for the use of ventilation to control atmospheric hazards within the tanks prior to and during entry, on or about 2/24/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
