29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about August 4, 2016, located at the southeastern portion of the building on the second floor, there were two floor holes that were approximately 24 inch by 48 inch where employees removed scrap material, thereby exposing the employees to a fall hazard of approximately 10 feet. b. On or about August 4, 2016, located at the eastern end of the 3rd floor, employees were operating cutting torches to remove a generator's motor while standing at an unprotected floor hole that was approximately 6 foot wide, thereby exposing the employees to an 11 foot fall hazard. c. On or about August 4, 2016, located at the southwestern portion of the 3rd floor, employees dropped scrap material through an 11 foot, 3 inch wide unguarded floor hole, thereby exposing the employees to a fall hazard of approximately 22 feet. d. On or about August 4, 2016, located where the eastern generator was located on the 3rd floor, there was a floor 7 foot 4 inch by 12 foot 4 inch floor hole where employees worked, thereby exposing the employees to a fall hazard of approximately 12 feet. e. On or about August 4, 2016, located where the western generator was located on the 3rd floor, there was a approximate 6 foot floor hole where employees were operating cutting torches, thereby exposing the employees to a fall hazard of approximately 12 feet. f. On or about August 4, 2016, located at the southeastern portion of the 3rd floor under the condenser tubes, employees dropped scrap material down into the basement without any protection from a fall hole that was approximately 25 feet long and 24 inches wide, thereby exposing the employees to a fall hazard of approximately 22 feet. g. On or about August 4, 2016, located at the 3rd floor walkway to the southern end of the building, there was a 34 inch by 70 inch floor hole where employees removed scrap material, thereby exposing the employees to a fall hazard of approximately 22 feet. h. On or about August 4, 2016, located at the southwestern stairway on the 4th floor, there was a 11 foot wide floor hole near the boiler that had no means to protect the employees from a fall, thereby exposing the employees to a fall hazard of approximately 10 feet. i. On or about August 4, 2016, located at the southwestern walkway to the 5th floor, there was a floor hole that measured 60 inches wide by 25 inches where employee gained access to the 5th floor, thereby exposing employees to a fall hazard of 11 feet. j. On or about August 4, 2016, located in the center of the 5th floor, there was an unprotected floor hole that measured 12 feet in diameter where employees worked, thereby exposing the employees to a fall hazard of 21 feet. k. On or about August 4, 2016, located at the southern end of the 6th floor, there was an unprotected open floor hole that was that was approximately 40 inches wide and 25 feet long which exposed the employees to a fall hazard of approximately 21 feet. l. On or about August 4, 2016, located at the 6th floor walkway to the middle set of stairs, there was an unprotected floor hole that was approximately 36 inches by 36 inches, thereby exposing the employees to a fall hazard of approximately 21 feet.
