29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. a. 4400 S. Packers - On or about September 23, 2014, operators were not evaluated every three years in the safe operation of powered industrial trucks, thereby exposing all employees in the plant to fractured bones, crushing injuries and fatal injuries due to struck by hazards. b. 4000 S.
