29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Employees were exposed to falls, lacerations, cuts and impalement hazards when adjusting and clearing material on the Old Shear sorting house conveyor without the use of locks and tags. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
