29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a. At the site, employees were exposed to a caught in hazard due to the inadvertent energization of equipment or machines, in that, employees were required to conduct maintenance on equipment and machines such as, but not limited to, two compressors, an alligator shear, two down stroke balers, a REB two ram Baler, a T 900 ton Shear, two forklifts, a Bobcat S-300, and a Sennebogen. Annual inspections of the company lockout tagout program were not conducted to assure that procedures were developed and implemented.
