29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite, 624-626 East Flora Street - Employees were performing stucco operations from a fabricated frame scaffold up to 20 feet above the ground level without being trained in scaffold safety. Employees did not know the trigger height for fall protection on fabricated frame scaffolds on or about 7/7/14. Penn Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.454(a), which was contained in OSHA inspection number 448073, citation number 1, item number 4b and was affirmed as a final order on 11/1/12, with respect to a workplace located at 1824 Christian Street Philadelphia, PA.
