29 CFR 1910.119(g)(3): The employer did not prepare a record which contained the identity of the employee, the date of training was completed, and the means of verifying that the employee had received and understood the training required by 29 CFR 1910.119(g): a)At the establishment, there was no documentation of training on specific in-plant operating procedures, operating limits, safety systems, emergency procedures, mechanical integrity program procedures for the inspection, testing and preventative maintenance of covered process equipment, safety controls, and safe work practices. b)At the establishment, there was no documentation of the means that the employer used to verify through demonstration that the employees understood any training they received including startup and shutdown. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
