29 CFR 1910.119(d)(3)(iii): For existing equipment designed and constructed in accordance with codes, standards, or practices that are no longer in general use, the employer did not determine and document that the equipment was designed, maintained, inspected, tested, and operating in a safe manner. a)On or about October 4, 2010 and times prior thereto, the employer did not document that stop valves were maintained in a safe manner in that, they were not suitably labeled. b)On or about October 4, 2010 and at times prior thereto, the employer did not document that the engine room was designed and maintained in a safe manner in that, the double door was not tight fitting. c)On or about October 4, 2010 and at times prior thereto, the employer did not document that the piping throughout the facility was maintained in a safe manner in that, it was not properly labeled to identify the refrigerant, physical state, relative pressure of refrigerant, and direction of flow. d)On or about October 4, 2010 and at times prior thereto, the employer did not document that the compressors were maintained and operated in a safe manner in that, the shaft alignment was not performed at least annually. e)On or about October 4, 2010 and at times prior thereto, the employer did not document that the engine room was designed and maintained in a safe manner in that, it was not equipped with a functioning ammonia alarm. Abatement Note: Among other methods to ensure that equipment is designed and operating in a safe manner the employer may comply with the following: label stop valves in accordance with 2010 ASHRAE-15; ensure engine room doors are tight fitting per 2010 ASHRAE-15; label piping in accordance with IIAR Bulletin 114; check compressor shaft alignment annually per IIAR Bulletin 110; ensure engine rooms are equipped with functioning ammonia detection alarms per 2008 IIAR-2.
