29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits before further use or in a safe and timely manner: (a)On or about 04/08/2008 - The Process Units within the Crude Unit Area - Safety Relief Valves PSV 601, PSV 602, and PSV 4 (KH-4) - The back pressure exceeded 10 percent of the set pressure allowable for the conventional type valve.
