29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: (a) On or about January 19, 2012, the maintenance employee performed trouble shooting/voltage testing on the 480 VAC on one of the Bruno presses and electrical safety-related work practices were not employed, such as the wearing of appropriate personal protective equipment when the task required electrical power to be present and could not be de-energized and locked out.
