29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about August 20, 2024, employees engaged in maintenance activities in the commercial bakery facility located at 4657 G Street, Omaha, Nebraska 68117 were exposed to the unexpected release of hazardous energy in that two employees were observed conducting maintenance operations on the robot portion of the Schubert robot located in the production area without affixing a lockout or tagout device.
